  • COVID Think Tank | Why India needs to be prepared for a COVID-19 third wave

    Irrespective of whether it comes or not, we need to prepare for a third wave of COVID, said renowned Critical Care Specialist Dr Brijendra Kumar Rao. Speaking at The New Indian Express’ virtual event COVID Think Tank, he pinpointed caution and preparation as the most important steps the country can take now. The critical care specialist was weighing in on the subject Critical Care: A New Architecture, and he stressed on the importance of learning from the past.

    “The problem is that the second wave was nothing like the first wave, so we weren’t able to deal with it the same way,” he says, “To go from a small number to over four lakh active cases is not something we could have prepared for in the same way. No country has dealt with a challenge that great. Now, we have these speculations of the third wave. The only thing we know is that we should be prepared for it whether it comes or not.”

    It’s not a question of where we went wrong, he says. “It is about being prepared for what is to come even if the challenges are different. A few months ago, we didn’t know what we were facing. But now, we have the experience of the first wave and second wave with us. If it is anything like the previous one, we should be able to sail through easily.”

    So, what did we learn from the second and more severe wave of COVID-19? The doctor believes that the medical infrastructure and the powers were able to take away an important lesson on building capacity. Rao calls for the timely training of medical students and allied health professionals.

    NEW DELHI:  Top authorities on Friday warned that a third Covid-19 wave could strike the country and peak higher if the restraints and measures being undertaken now to control the second wave of the infectious disease are dropped or eased without caution.

    In a press briefing by the Union health ministry, VK Paul, member, health, Niti Aayog who heads the country’s Covid-19 national task force underlined that the pandemic situation had not improved because of the virus behaviour but because of the measures taken to curb its spread.

    “If we again start doing what we were doing as a society in December, January, the situation can again go into a difficult phase,” he said. “More of that (not maintaining Covid appropriate behaviour) we do, higher will be the wave but if we proceed slowly and continue the tightrope walk, the wave will be small..maybe there will be no waves at all,” he said.

    The figures shared by the government on Friday showed that there has been a 68 % decline in daily cases from May 7.

    Also, around 377 districts are reporting less than five % test positivity rate and 257 districts are reporting more than 100 daily Covid-19 cases—all signs that the pandemic is now under control in many parts in India.

    Paul however insisted that this is not happening automatically. “ There are no crowds.. a price that we pay but we have made it difficult for the virus to travel. But we have to remember that when the peak is declining, if we start doing what we were doing in January, it will come back again. This is mathematically valid and also valid by common sense,” he stressed.

    Many experts have said that while a third wave of the pandemic is a real threat and could occur given the still large number of people who may be susceptible to the virus, it is difficult to predict its timing and intensity.

    Some doctors have even predicted that the third wave, when it arrives, maybe even tougher for children who have largely been unaffected so far and may be left completely uncovered through vaccination coverage but the government authorities and other experts have dismissed these concerns.

    Paul, however, maintained that the government was preparing for the worst-case scenario and guidelines on upgrading medical infrastructure for kids will soon be shared with states.

Doctors also said, AIIMS can increase the emergency related medical system as per the requirement, which has also been prepared. He said, AIIMS has enough medicine and necessary injection system for black and white fungus. Doctors informed Vikas Upadhyay that the number of infected patients of the second wave has reduced to a great extent. Most ICUs and ventilators are empty, despite our preparations for any kind of new infected. Vikas Upadhyay thanked the doctors of AIIMS and praised his contribution in this transition period.

    KOCHI/BENGALURU/DELHI/JAIPUR/MUMBAI:  While the Supreme Court directed the Centre to prepare for the third wave, the second wave itself was nowhere close it its peak, with the fresh case count on Thursday crossing the four-lakh-mark for the second time since April 30.

    Kerala, which accounted for over 10% of the nation’s tally, announced a rigourous nine-day complete lockdown till May 16, starting Saturday, as it recorded its highest single-day spike of 42,464 and 63 deaths.

    While Karnataka’s oxygen situation is yet to be resolved, the Centre challenged in the Supreme Court an order of the Karnataka High Court directing it to raise the state’s oxygen share. In Delhi, the city government was hauled over the coals by the high court after it objected to an observation that the existing medical infrastructure was ‘exposed’ and in ‘shambles’ when put to the test.

    The court ordered the Delhi government to provide medical treatment to all people suffering from Covid-19 in the city, which saw an addition of 19,133 new cases and 335 deaths. It got over 730 MT oxygen for the first time. Rajasthan, too, opted for a complete lockdown from May 10 to May 24 to flatten the curve. After almost a week of reporting less than 60,000 cases, Maharashtra’s tally increased by 62,194 on Thursday.

    AIIMS director Dr. Nitin M. Nagerkar said that after the second wave, most of the serious patients are coming to AIIMS. In such a situation, it becomes challenging to bring them back to normalcy. He said that most patients are in need of oxygen and steroid immediately. AIIMS has provided adequate oxygen beds for this. He also apprised MP Soni about vaccination and availability of oxygen.
