Bihar requires students admitted to MD or MS programmes through NEET‑PG to serve the state’s health system for three years after graduation. The rule is backed by a financial bond of Rs 25 lakh, which must be paid if the doctor exits the service early. The bond also includes repayment of any scholarship received during the postgraduate course.

If a candidate abandons the MD/MS programme before completion, a lower penalty of Rs 15 lakh applies, along with the return of the scholarship.

Colleges collect the bond at the time of admission and retain the MBBS certificate as collateral until the service obligation is fulfilled or the amount is settled. The policy aims to strengthen specialist staffing in district hospitals and other government health facilities across Bihar.
